All students are expected to be in school on required student attendance days. If your child is absent, a parent needs to provide either a written notice or call the school attendance line. If these conditions are met, the absence will be considered "excused" on the student's report card. Please note, if a notice or call is not received, the absence will be considered "unexcused". According to CPS Promotion Policy, any student in grades 3 or 6 with more than 10 unexcused absences will be required to attend summer school despite a student's grades and/or test scores.
A Parental Reminder: Students may be legitimately absent for the following reasons ONLY:
• Student illness
• Death in the immediate family
• Religious holiday
• Family emergency
• Circumstances causing reasonable concern to the parent for the student’s health or safety, or
• Other situations beyond the control of the student, as determined by the principal.
Daily Time Schedule
7:45am - 2:45pm
Students are discouraged from arriving at school more than 15 minutes before classes begin and from lingering after-school, unless they are attending an after-school activity.
Students are tardy at 7:55 a.m. If students arrive 61 minutes past school start time ( 8:46 a.m. ) they will be considered 1/2 day.
All children dismissed early must be picked up in the office. Students dismissed before 2:15pm are considered 1/2 day. Students will be released to individuals whose names are listed on the authorization for early release form completed by parent or guardian.
Regular and prompt school attendance is extremely important. Please make every effort to see that your child attends school each day he/she is able. An ill child, however, should not be in school. This may result in his/her health being impaired further and my expose other children to a communicable disease.
The school must have on file two phone numbers where parents can be reached during the school day.
In case of absence due to certain communicable disease, a student cannot be readmitted to school without written permission from a physician.

Please make sure that your child has an updated emergency and medical form.
Daily Arrival Procedures
Students arriving at 7:30 a.m. (the earliest arrival time) will enter the main entrance and proceed to the gym. Students arriving at school between 7:45 a.m. to 7:55 a.m. can go directly to their classroom. All students arriving after the 7:55 bell must report to the office for a tardy slip /pass.
The school fee for the 2015-2016 school year is $100 for families with one child at McDade / and an additional $25 for families with more than one child at McDade. These fees help to support our student incentives programs. McDade will accept cash and money order payments only.
Classroom supply list will be provided by the classroom teacher and is also available under student on our website. We ask that children bring to school only those items directly related to their work. Students must refrain from bringing music players, toys, games and other non-educational items. Any items discovered at the school will be confiscated and only be given back to the parent.
The Chicago Public School Discipline Code Handbook informs students, parents, teachers, and administrators about the Board’s expectation of student behavior. It describes consequences for unacceptable student behavior in our school.
The Chicago Public School rules, regulations, and standards or discipline balance the rights of the individual student with the right of the school society.
Students work and function in a school community and as community members, are obligated to behave in a way that fosters mutual respect and cooperation. This is necessary so the rights, benefits, and opportunities of all school community members may be protected and enjoyed.
If you enrolled your child for transportation, you should be receiving routing information before the start of the school year. If you want transportation, please call the main office to obtain an application.
Students are required to wear a McDade gym uniform. Gym uniforms are ordered by our school PTA. Order forms will be sent home in the middle of September.
Asthma/Food Allergies: If you child has asthma or food allergies, please let the school know immediately so that we can be aware and create a plan for your child.
In compliance with the CPS Healthy Foods and Beverage Policy, parents may not bring dessert items (i.e. cakes, cupcakes) for an entire class for birthday celebrations.
Dental Exam: All students in kindergarten, 2nd, and 6th grade must have a dental exam completed by a licensed dentist prior to May 15 of the current school year.
Immunization/Physical Waiver: If you have objections against immunizations due to personal reasons (religious reasons, etc.), you must complete an immunization waiver form. This form is available in the main office.
Physicals: Students entering kindergarten and 6th grade must have a physical examination before the first day of school.

Vision Exam: All students in kindergarten, 2nd grade, and any students new to Chicago Public Schools, must have a vision screening/exam before October 15th.
